[CCIT] Model Contracts-Personal Service, Grant, and Interagency & Revised Signature Pages-OSC Website

All,

 

Tomorrow, by close of business, all of the old model contracts and
related instructions from the OSC/CCU website (excepting the one
entitled Escrow Contract and Security Agreement) will be replaced with
the attached models. These models are duplicates of those being loaded
in the new Cobblestone contract management system.

 

In addition, all of the Signature pages have been revised to add an
averment by the person signing on behalf of the Agency or IHE regarding
Statutory Violations. The new versions of the signature pages will also
be on the website. The purpose of this averment is to obviate the
necessity of sending a Late Memo for contracts that contain the
averment.

 

For all contracts currently in process, you may continue to with the old
models and signature pages or use the new ones; it is your choice.
However, the new models and signature pages should be used for all new
contracts after tomorrow.
